After filling the form, scan all the pages (PDF, Word Doc, Images) and send them through via email to: director@portlanddaycare.com or you can drop it off when the centre is open.

Please note: you will receive a generic confirmation email stating that your email has come through. That email’s sole purpose is a confirmation of receiving your child’s application (provided the right documents are attached). By no means is it a confirmation of your child’s enrollment.
